Summary

Endoscopic fenestration of the septum pellucidum offers a less invasive treatment for monoventricular hydrocephalus. This neuroendoscopic technique effectively resolves intracranial hypertension and reduces ventricle size in all patients.

Area of Science:

  • Neurosurgery
  • Pediatric Neurosurgery
  • Minimally Invasive Neurological Procedures

Background:

  • Monoventricular hydrocephalus traditionally treated with extrathecal shunting.
  • Endoscopic fenestration of the septum pellucidum presents a less invasive alternative.
  • This technique is particularly useful for managing complex hydrocephalus cases.

Purpose of the Study:

  • To evaluate the efficacy of endoscopic fenestration of the septum pellucidum in treating monoventricular hydrocephalus.
  • To describe a neuroendoscopic approach for septostomy in hydrocephalus management.
  • To assess the outcomes of this minimally invasive technique.

Main Methods:

  • Neuroendoscopic fenestration of the septum pellucidum performed in five patients.

Related Experiment Videos

  • In cases of excluded lateral ventricles, initial cannulation of the normal ventricle was performed.
  • Septostomy was executed from the normal to the enlarged lateral ventricle.
  • Main Results:

    • Complete remission of intracranial hypertension symptoms observed in all five patients.
    • Significant decrease in the size of the enlarged ventricle noted across all cases.
    • Successful endoscopic fenestration achieved with a direct trajectory.

    Conclusions:

    • Endoscopic fenestration of the septum pellucidum is the preferred treatment for monoventricular hydrocephalus.
    • Recommends initial cannulation of the normal lateral ventricle for septostomy.
    • Effective even in cases with choroid plexus cysts obstructing the foramen of Monro.
