Summary

Repaglinide effectively lowers blood glucose and HbA1c in Type 2 diabetes patients by stimulating glucose-dependent insulin release. It demonstrates comparable efficacy to sulfonylureas with a similar safety profile regarding hypoglycemic events.

Background:

  • Type 2 diabetes mellitus (T2DM) is characterized by deficient physiological insulin secretion.
  • Repaglinide, a benzoic acid derivative, enhances insulin release from pancreatic beta-cells.
  • This insulin release is specifically stimulated by the presence of glucose.

Purpose of the Study:

  • To establish the effective dose range of repaglinide in T2DM patients.
  • To evaluate the efficacy and safety of repaglinide monotherapy compared to placebo and sulfonylureas.

Main Methods:

  • Two placebo-controlled studies assessed dose-ranging (0.25-4.0 mg and 0.25-8.0 mg) over 4-16 weeks.
  • Phase III trials compared repaglinide (0.5-4.0 mg) with glibenclamide, glipizide, and gliclazide over 1 year.
  • Outcomes included fasting plasma glucose, 2-h postprandial glucose, HbA1c, and hypoglycemic events.

Main Results:

  • Repaglinide significantly decreased fasting and postprandial blood glucose compared to placebo (p < 0.05 to p < 0.001).
  • Glycated hemoglobin (HbA1c) decreased from 8.5% to 7.9% with repaglinide, while placebo increased from 8.1% to 9.2% (p < 0.001).
  • Repaglinide showed superior efficacy to glipizide and equivalent efficacy to glibenclamide and gliclazide regarding HbA1c changes. Hypoglycemic event rates were similar across treatments (16% vs. 15-20%).

Conclusions:

  • Repaglinide monotherapy, combined with diet and exercise, is an effective treatment for Type 2 diabetes.
  • Repaglinide provides significant glycemic control and is a viable alternative to sulfonylureas.
