Summary

This study reports the first case of an acetabular pneumatocyst with an air-fluid level in a patient with ankylosing spondylitis. This finding expands understanding of intraosseous gas manifestations in joint disease.

Area of Science:

  • Radiology
  • Orthopedic Surgery
  • Rheumatology

Background:

  • Intraosseous gas is typically associated with osteomyelitis, vacuum phenomenon, or post-surgical/traumatic conditions.
  • Subchondral gas-filled lesions, termed pneumatocysts, have been documented in the sacroiliac joint and clavicle, but without intralesional air-fluid levels.
  • The etiology of these pneumatocysts remains uncertain, and they are generally considered benign.

Observation:

  • A unique case of an acetabular pneumatocyst containing an air-fluid level is presented.
  • The patient was a 62-year-old male with a history of long-standing ankylosing spondylitis affecting the hip joint.

Findings:

  • This represents the first documented instance of a pneumatocyst in the acetabulum exhibiting an air-fluid level.

Related Experiment Videos

  • The presence of an air-fluid level within the acetabular pneumatocyst is a novel radiological finding.
  • Implications:

    • This case expands the spectrum of known intraosseous gas presentations, particularly in the context of chronic arthropathies like ankylosing spondylitis.
    • Further investigation may elucidate the specific mechanisms leading to air-fluid level formation in these lesions.
    • Radiologists and clinicians should consider this presentation when evaluating hip joint abnormalities in patients with spondyloarthropathies.
