Background:

  • Membranous fat necrosis (MFN) is a degenerative adipose tissue process characterized by membranocystic lesions (MCLs).
  • The pathogenesis of MFN remains largely unknown, with ischemia from atherosclerosis or venous insufficiency being the most commonly suspected causes.
  • Systemic vasculitis has been infrequently cited as a potential ischemic basis for MFN, but direct evidence in tissue has been lacking.

Observation:

  • This study reports MFN in association with three uncommon forms of vasculitis.
  • Observed cases include granulomatous vasculitis in a post-herpetic scar, cytomegalovirus-induced vasculitis in systemic lupus erythematosus, and lymphocytic vasculitis at a tetanus toxoid immunization site.

Findings:

  • The findings demonstrate a direct link between specific types of vasculitis and the development of MFN.
  • Vasculitis, particularly uncommon forms, should be considered in the differential diagnosis of MFN.
  • Histopathological examination of MFN lesions can reveal associated vasculitic changes.

Implications:

  • This research expands the understanding of MFN pathogenesis beyond traditional ischemic mechanisms.
  • Identifying vasculitis as a cause of MFN may lead to earlier diagnosis and targeted treatment strategies.
