Summary

Conventional-dose chemotherapy for testicular diffuse large-cell lymphoma shows poor outcomes, with high rates of progression and death. Novel treatment strategies are needed to improve survival and control central nervous system and contralateral testis disease.

Area of Science:

  • Oncology
  • Hematology

Background:

  • Testicular diffuse large-cell lymphoma (TDLCL) is a rare non-Hodgkin lymphoma.
  • Conventional-dose chemotherapy has shown limited efficacy in TDLCL.

Purpose of the Study:

  • To evaluate the clinical outcomes of patients with TDLCL treated with conventional-dose systemic chemotherapy.
  • To identify prognostic factors and patterns of relapse in TDLCL.

Main Methods:

  • Retrospective analysis of 29 adult patients with TDLCL treated with doxorubicin-based chemotherapy.
  • Patients were classified by stage (Ann Arbor I-IV) and International Prognostic Index.

Main Results:

  • Median follow-up was 82 months. Disease progression occurred in 22 patients, and 22 patients died.

Related Experiment Videos

  • Actuarial median survival was 41 months for limited stage and 16 months for advanced stage.
  • Relapse occurred in 14 patients, with 41% involving the CNS or contralateral testis.
  • Conclusions:

    • Conventional-dose chemotherapy offers a poor prognosis for TDLCL patients.
    • More effective strategies, including high-dose chemotherapy and CNS-directed treatment, are needed.
    • Contralateral testicular irradiation may be considered in treatment planning.
