Background:

  • Warfarin is a common anticoagulant used for stroke prophylaxis in patients with atrial fibrillation.

Observation:

  • Two patients on stable warfarin therapy experienced elevated international normalized ratios (INRs) after starting tolterodine.
  • INR levels returned to baseline after tolterodine discontinuation, with no changes in warfarin dosage.

Findings:

  • The initiation of tolterodine was temporally associated with a significant increase in INR in both patients.
  • No other confounding factors were identified that could explain the elevated INR.
  • The drug interaction appears reversible upon tolterodine cessation.

Implications:

  • Clinicians should exercise caution and monitor INR closely when initiating tolterodine in patients stabilized on warfarin.
  • This case series suggests a potential drug interaction between tolterodine and warfarin, necessitating vigilance.
  • Further research into the mechanisms of this interaction is warranted.
