Summary

Monocyte function, measured by HLA-DR expression, is reduced in acute pancreatitis patients. This immune marker does not recover in non-survivors, indicating its prognostic value for severe acute pancreatitis.

Area of Science:

  • Immunology
  • Gastroenterology
  • Critical Care Medicine

Background:

  • Acute pancreatitis involves complex inflammatory responses.
  • The role of the monocyte/macrophage system in pancreatitis pathogenesis is not fully understood.
  • Assessing monocyte function may offer prognostic insights.

Purpose of the Study:

  • To investigate the involvement of the monocyte/macrophage system in acute pancreatitis.
  • To correlate monocyte function, assessed via HLA-DR expression, with patient outcomes.

Main Methods:

  • Prospective clinical study involving 37 acute pancreatitis patients at a German university clinic.
  • Monocyte function was measured by human leukocyte antigen – antigen D related (HLA-DR) expression.
  • Patients were stratified into mild pancreatitis, severe pancreatitis with survival, and severe pancreatitis with mortality groups.

Related Experiment Videos

Main Results:

  • HLA-DR expression was initially depressed in all pancreatitis groups.
  • Significant recovery of HLA-DR expression was observed in mild pancreatitis patients by day 3 (p < 0.05).
  • A significant difference in HLA-DR expression emerged by day 7 between severe pancreatitis survivors and non-survivors (p < 0.05).

Conclusions:

  • Monocyte function, indicated by HLA-DR (CD14+DR+) expression, is impaired in acute pancreatitis.
  • Lack of HLA-DR expression recovery is associated with mortality in severe acute pancreatitis.
  • Reduced HLA-DR expression serves as a potential biomarker for predicting outcomes in acute pancreatitis.
