Summary

A rare case of an aneurysmal portosystemic venous shunt was successfully treated. This condition, causing fatigue and tachycardia, was identified using colour Doppler ultrasound and closed with detachable coils.

Background:

  • Portosystemic venous shunts are abnormal connections between the portal venous system and systemic circulation.
  • Aneurysmal variations of these shunts are exceptionally rare, posing diagnostic and therapeutic challenges.

Observation:

  • A young female presented with postprandial fatigue and paroxysmal tachycardia.
  • Colour Doppler ultrasound (CDUS) revealed an aneurysmal direct vascular communication between the right portal vein and the right hepatic vein.

Findings:

  • CDUS accurately identified the intrahepatic portosystemic venous shunt.
  • Direct portal angiography confirmed the shunt's anatomy.
  • Complete occlusion of the shunt was achieved using detachable coils.
