Summary

This study evaluated common bile duct stone removal techniques, finding laparoscopic procedures more efficient and cost-effective than traditional open surgery or endoscopic retrograde cholangiopancreatography (ERCP). Surgeons should adopt minimally invasive laparoscopic methods for better patient outcomes.

Area of Science:

  • Gastroenterology
  • Surgical Innovation
  • Biliary Tract Surgery

Background:

  • Common bile duct stones pose a significant clinical challenge.
  • Traditional management often involves open choledochotomy or endoscopic retrograde cholangiopancreatography (ERCP) with endoscopic sphincterotomy (EST).
  • Laparoscopic techniques are emerging as a less invasive alternative.

Purpose of the Study:

  • To compare the efficacy and outcomes of different surgical approaches for common bile duct stone removal.
  • To evaluate the learning curve and challenges associated with intraoperative video-choledochoscopy.
  • To advocate for the adoption of laparoscopic common bile duct stone extraction.

Main Methods:

  • Retrospective analysis of 92 patients undergoing open common bile duct exploration.

Related Experiment Videos

  • One patient underwent laparoscopic common bile duct exploration.
  • Intraoperative video-choledochoscopy was utilized in 16 cases.
  • Postoperative investigations included cholangiography and ERCP.
  • Main Results:

    • Open choledochotomy was performed when endoscopic approaches failed or stones were found intraoperatively.
    • Intraoperative video-choledochoscopy increased operative time by 47 minutes and was associated with residual stones in 4 patients, likely due to surgeon inexperience.
    • Postoperative interventions were required in several cases, with varying success rates.

    Conclusions:

    • Laparoscopic common bile duct stone removal, utilizing instruments like guide-wires and Dormia baskets, offers a successful, comfortable, and cost-saving approach.
    • Mastery of laparoscopic techniques and associated instrumentation is recommended for surgeons.
    • Minimally invasive, one-stage laparoscopic procedures are associated with high success rates in the literature.
