Summary

Midface distraction surgery successfully advanced the midface in a 10-year-old with bilateral cleft lip and palate. This distraction osteogenesis improved facial aesthetics and dental occlusion without affecting speech.

Background:

  • Bilateral cleft lip and palate often results in midface hypoplasia.
  • Surgical correction aims to improve facial aesthetics, dental occlusion, and function.

Observation:

  • A 10-year-old patient with bilateral cleft lip and palate underwent high LeFort I osteotomy and Rigid External Distraction (RED) halo fixation.
  • Maxillary advancement of 17 mm was achieved at a rate of 1-1.5 mm/day, starting five days post-surgery.

Findings:

  • The patient experienced improved facial aesthetics and dental occlusion, with a temporary overcorrection to a Class III relationship.
  • Velopharyngeal function remained unaffected throughout the treatment and follow-up period.
  • Nine-month follow-up showed stable results with no complications.
