Summary

Soluble E-selectin is elevated in cutaneous lupus erythematosus (CLE) patients with active skin lesions, unlike systemic lupus erythematosus (SLE). This suggests endothelial cell involvement and potential E-selectin targeted therapies for CLE.

Background:

  • Soluble adhesion molecules (sCAMs) like sE-Selectin, sICAM-1, and sVCAM-1 are elevated in inflammatory diseases, including systemic lupus erythematosus (SLE).
  • Previous studies have not consistently reported elevated sCAMs in cutaneous lupus erythematosus (LE).

Purpose of the Study:

  • To investigate the levels of sE-Selectin, sICAM-1, and sVCAM-1 in patients with different forms of lupus erythematosus (LE).
  • To explore the correlation between these sCAMs and disease activity, particularly in cutaneous LE.

Main Methods:

  • Serum samples from 51 LE patients (24 SLE, 19 DLE, 8 SCLE) and 32 controls were analyzed using ELISA kits.
  • Serial samples from 12 LE patients were used to assess changes over time and disease activity.

  • Significantly elevated sE-Selectin levels were observed in patients with discoid lupus erythematosus (DLE) and widespread lesions.
  • SLE and subacute cutaneous LE (SCLE) patients did not show elevated sE-Selectin, but had increased sICAM-1 and sVCAM-1.
  • sE-Selectin levels correlated with active skin lesions in serial samples, whereas sICAM-1 remained elevated regardless of disease activity.

Conclusions:

  • The findings suggest a more significant role for endothelial cell activation in cutaneous LE than previously recognized.
  • Selective inhibition of E-selectin may hold therapeutic potential for managing cutaneous LE.
