Recall accuracy for screening mammograms differs significantly between care systems and ethnic groups. This highlights potential biases in studies comparing different populations.

Previous research on screening mammogram recall accuracy often involved homogeneous populations.
Limited data exist on systematic group differences in recall accuracy.
This study addresses the need to evaluate recall accuracy across diverse healthcare systems.

Purpose of the Study:

To evaluate women's recall accuracy for screening mammogram behavior in two distinct care systems.
To identify potential systematic differences in recall accuracy between different patient groups.

Main Methods:

Women aged 50-70 from a military care system (MCS) and a county-funded system (CFS) were surveyed.
Mammogram status was verified using fiscal, clinic, and radiology records.

Participants were interviewed by blinded personnel regarding their mammogram history.

Main Results:

Screening mammogram recall sensitivity varied significantly between the MCS (95%) and CFS (79%).
Euro-American and Mexican American subgroups showed significant differences in sensitivity (97% vs. 80%).
Differences were also observed in the proportion of true negatives and educational attainment between groups.

Conclusions:

Systematic differences in recall validity exist between healthcare systems and demographic groups.
These disparities can compromise the accuracy of intergroup comparisons in health behavior research.
Findings underscore the importance of considering system and group variations in recall accuracy.
