Summary

Octreotide effectively treated severe postprandial orthostatic hypotension in a multiple system atrophy patient. This treatment, particularly octreotide, offers a potential new option for managing dysautonomia-related hypotension.

Background:

  • Multiple system atrophy (Shy-Drager syndrome) can cause severe symptomatic postprandial orthostatic hypotension.
  • Standard treatments like fludrocortisone and midodrine may become progressively ineffective.

Observation:

  • A patient with multiple system atrophy and severe orthostatic hypotension refractory to conventional therapies was treated with subcutaneous octreotide.
  • The patient reported clear beneficial effects, which were objectively confirmed by tilt table testing.

Findings:

  • Octreotide administration led to significant improvement in orthostatic hypotension symptoms.
  • Tilt table tests demonstrated the efficacy of octreotide in managing blood pressure during postural changes.
