Background:

  • A nurse-led domiciliary follow-up service was evaluated for patients with COPD exacerbations.
  • This service involved hospital assessment followed by home-based care.

Findings:

  • Hospital assessment combined with nurse-led domiciliary follow-up is a suitable management strategy for select COPD exacerbation patients.
  • The service demonstrated high acceptability among both patients and general practitioners (GPs).

Implications:

  • This model of care may reduce unnecessary hospital admissions for COPD exacerbations.
  • Implementing nurse-led community services can improve patient and clinician satisfaction in COPD management.
  • Findings support the integration of community-based nursing services into COPD care pathways.
