Background:

  • Preeclampsia is a serious pregnancy complication characterized by high blood pressure.
  • Retinal detachment is an uncommon but significant complication associated with preeclampsia.

Observation:

  • A 31-year-old woman experienced bilateral serous retinal detachment postpartum, two days after delivery.
  • Initial ophthalmic examination revealed no hypertensive retinopathy, but residual retinal pigment epithelium changes suggested healed choriocapillaris infarcts.

Findings:

  • The subretinal fluid spontaneously resorbed within days, leading to improved visual acuity.
  • Pathogenesis is linked to retinal and choroidal vascular damage, with fluorescein angiography supporting choroidal vascular damage as the primary cause of detachment.

Implications:

  • Conservative management is recommended for retinal detachment in preeclampsia cases.
  • Retinal detachment in preeclampsia may serve as an indicator for pregnancy termination.
  • Early recognition and conservative treatment lead to favorable visual outcomes.
