Background:

  • Hypersensitivity pneumonitis (HP) is an immune-mediated lung disease caused by inhaled antigens.
  • Pigeon breeder's disease (PBD) is a common form of HP, typically seen in adult pigeon fanciers.
  • Pediatric HP is rare, and PBD in children is even less commonly reported.

Observation:

  • Two boys, aged 13 and 7, presented with chronic respiratory and systemic symptoms including coughing, dyspnea on exertion, anorexia, weight loss, and fatigue.
  • Initial diagnosis in both cases was asthma.
  • Clinical presentation, laboratory results, and radiographic findings were re-evaluated.

Findings:

  • A definitive diagnosis of pigeon breeder's disease was established in both pediatric patients.
  • The diagnosis was supported by a comprehensive review of anamnestic, clinical, laboratory, and radiographic data.
  • Both patients demonstrated complete recovery following appropriate medical treatment and strict avoidance of pigeon antigen exposure.

Implications:

  • This case series highlights the importance of considering PBD in the differential diagnosis of pediatric respiratory illnesses, even in the absence of typical risk factors.
  • Prompt diagnosis and antigen avoidance are critical for successful management and resolution of PBD in children.
  • Increased awareness among clinicians is necessary to prevent misdiagnosis and delayed treatment of this potentially serious condition in pediatric populations.
