Area of Science:

  • Gynecologic Oncology
  • Pathology

Background:

  • Malignant transformation of endometriosis is a documented phenomenon.
  • Endometrioid adenocarcinoma is the most common malignancy in this context.
  • Other carcinomas and stromal tumors are rarely observed.

Observation:

  • A 42-year-old woman with a history of aggressive endometriosis presented with recurrent vaginal masses.
  • She underwent multiple treatments, including radiotherapy for bleeding vaginal endometriosis.
  • Pathologic confirmation of recurrent endometriosis was made multiple times over four years.

Findings:

  • The first reported case of adenosarcoma, a rare mixed Mullerian tumor, arising in extrauterine vaginal endometriosis.
  • The adenosarcoma contained heterologous elements.

Related Experiment Videos

  • Surgical excision of a recurrent vaginal mass revealed the adenosarcoma.
  • Implications:

    • Biopsy or excision of recurrent endometriosis is crucial for early detection of malignant transformation.
    • This case expands the spectrum of known malignancies arising in endometriosis.
    • Prompt diagnosis and management are essential for improved patient outcomes in rare gynecologic malignancies.
