Summary

Macrovascular disease, including carotid artery and peripheral arterial disease, is significantly more common in patients with systemic sclerosis (SSc). Early screening may help identify at-risk individuals for cardiovascular mortality.

Background:

  • Systemic sclerosis (SSc) is associated with increased cardiovascular mortality.
  • The prevalence of macrovascular disease in SSc patients compared to controls is not well-established.

Purpose of the Study:

  • To investigate the prevalence of macrovascular disease in patients with systemic sclerosis (SSc).
  • To compare the incidence of asymptomatic peripheral vascular disease in SSc patients versus unaffected controls.

Main Methods:

  • 54 SSc patients and 43 controls underwent conventional atherosclerotic disease risk factor screening.
  • Non-invasive vascular assessments included carotid duplex scanning and ankle-brachial blood pressure index measurement.
  • Peripheral vascular disease was identified using these non-invasive methods.

Main Results:

  • Carotid artery disease was found in 64% of SSc patients versus 35% of controls (p=0.007).
  • Peripheral arterial disease was present in 17% of SSc patients, compared to 0% of controls (p=0.003).
  • No significant differences in conventional risk factors like smoking or cholesterol were observed between groups.

Conclusions:

  • Macrovascular disease is significantly more prevalent in patients with systemic sclerosis.
  • Screening SSc patients for macrovascular disease could enable early identification of high-risk individuals.
  • Further research into treatments to reduce cardiovascular mortality in SSc is warranted.
