Background:

  • Elective graft repair for infrarenal aortic aneurysm (AAA) and aortoiliac occlusive disease (AIOD) traditionally uses standard transabdominal (STA) approaches.
  • Minimally invasive techniques aim to reduce patient morbidity and hospital resource utilization.

Purpose of the Study:

  • To assess the clinical outcomes of a minilaparotomy technique (MLT) for elective infrarenal AAA and/or AIOD repair.
  • To compare MLT outcomes with those of the standard transabdominal (STA) approach.

Main Methods:

  • Prospective, nonrandomized study comparing MLT (n=23) with STA (n=21) for infrarenal AAA or aortofemoral bypass.
  • Exclusion of patients requiring concomitant major vascular revascularization.

    • Morbidity and mortality rates were comparable between the two groups.

    Conclusions:

    • Minilaparotomy technique for elective aortic aneurysm repair leads to reduced ICU and hospital stays and decreased postoperative ileus.
    • MLT is a cost-effective alternative, avoiding expensive equipment and extensive postoperative surveillance required for endovascular repairs.
