Background:

  • BK virus (BKV) is a human polyomavirus.
  • BKV is usually asymptomatic but can cause symptomatic infections in immunocompromised individuals, such as renal transplant patients.

Observation:

  • A 14-year-old boy with acquired immunodeficiency syndrome (AIDS) presented with a rare BKV infection.
  • The infection affected the lung and kidney, leading to diffuse alveolar damage and death.

Findings:

  • Histopathology revealed large, homogenous purple intranuclear inclusions in infected type II pneumocytes and tubular epithelial cells.
  • Electron microscopy identified 40-nm intranuclear viral particles consistent with BKV.
  • In situ hybridization confirmed the diagnosis of BKV infection.
