A rare case of pulsatile perineal hematoma after prostate biopsy was successfully treated. Transcatheter embolization effectively closed the pseudoaneurysm, resolving the patient's symptoms.

Area of Science:

  • Urology
  • Interventional Radiology
  • Vascular Surgery

Background:

  • Transrectal biopsy of the prostate is a common procedure.
  • Complications, though rare, can include vascular injury and hemorrhage.
  • Pulsatile perineal hematoma is an uncommon but significant complication.

Observation:

  • A patient presented with a pulsatile perineal mass and sensation following a prostate biopsy.
  • Diagnostic imaging, including echo Doppler, CT, and arteriography, identified a pseudoaneurysm of the left hypogastric artery.

Findings:

  • Transcatheter intraarterial embolization using Gianturco-Wallace coils was performed to treat the pseudoaneurysm.
  • The procedure resulted in immediate reduction of aneurysm filling and complete resolution of the patient's symptoms.

Related Experiment Videos

Implications:

  • Transcatheter embolization is an effective and minimally invasive treatment for pelvic hemorrhage secondary to pseudoaneurysms.
  • This technique offers a low morbidity and mortality rate, making it a valuable option for managing such complications.
