Background:

  • Tissue Doppler imaging (TDI) is a novel non-invasive technique for assessing myocardial relaxation velocities.
  • Quantitative assessment of diastolic ventricular function in fetuses is crucial for understanding cardiac development.
  • Normative data for fetal myocardial tissue velocities are currently lacking.

Purpose of the Study:

  • To establish normative data for myocardial tissue velocities in normal fetuses using TDI.
  • To investigate the relationship between gestational age and myocardial velocities in fetal ventricles.
  • To explore correlations between TDI and pulsed Doppler measurements of diastolic function.

Main Methods:

  • Longitudinal measurements of left ventricular posterior wall, right ventricular anterior wall, and interventricular septum velocities were performed in 30 normal fetuses (19-38 weeks gestation).
  • Peak myocardial velocities during early diastole (EW) and atrial contraction (AW) waves were recorded.
  • Data were analyzed for normative values, gestational age-related changes, and correlations with pulsed Doppler parameters.

Main Results:

  • Normative mean values for EW and AW were established for fetal left and right ventricles.
  • Interventricular septum velocities (EW, AW) were significantly lower than ventricular walls (P<0.01).
  • EW and E/AW significantly increased with gestational age in all measured walls (P<0.01), suggesting maturational changes. Significant correlations were found between TDI E/AW and pulsed Doppler E/A (P<0.01).

Conclusions:

  • Fetal myocardial tissue velocity assessment using TDI is feasible.
  • Established normative velocity data and observed age-related increases suggest TDI can detect maturational changes in fetal diastolic function.
  • TDI provides a valuable tool for quantitative evaluation of fetal cardiac diastolic function and development.
