Background:

Purpose of the Study:

  • To evaluate the safety and efficacy of sub-Tenon's parabulbar anesthesia for posterior segment surgery.

Main Methods:

  • Prospective case series of 200 patients undergoing posterior segment surgery.
  • Primary anesthesia administered via sub-Tenon's parabulbar injection (lidocaine, bupivacaine, hyaluronidase).
  • Supplementation rates and complications were monitored, analyzing relation to surgery duration and pain level.

Main Results:

  • 14% of patients required supplemental anesthesia (intravenous or intraoperative local).
  • Supplementation was significantly associated with longer surgery duration (>3 hours) and more painful procedures (scleral buckle, cryotherapy).

  • Sub-Tenon's parabulbar anesthesia is safe and effective for shorter (<3 hours) posterior segment surgeries like vitrectomy.
  • More extensive or painful procedures may necessitate supplemental anesthesia for optimal patient comfort and surgical conditions.
