Hepatocyte transplantation in acute liver failure

Background:

• Acute liver failure (ALF) has a high mortality rate, with orthotopic liver transplantation (OLT) being the only definitive treatment.
• Limited donor organs for OLT necessitate alternative therapeutic strategies for ALF patients.

Purpose of the Study:

• To evaluate the safety and feasibility of hepatocyte transplantation (HT) in patients with severe ALF.
• To assess the engraftment and function of transplanted hepatocytes in ALF recipients.

Main Methods:

• Cryopreserved human hepatocytes were assessed for viability and function.
• Five severe ALF patients underwent intrasplenic and/or intrahepatic HT under cyclosporine immunosuppression.
• Engraftment was confirmed using histology and fluorescent in situ hybridization (FISH).

• Three of five patients showed significant improvement in encephalopathy, ammonia levels, and prothrombin times.
• Enhanced clearance of aminopyrine and caffeine indicated restored liver function.
• Postmortem analysis confirmed hepatocyte engraftment in the liver and spleen.

• Hepatocyte transplantation is a feasible, albeit complex, option for severe ALF patients.
• While complications are present, HT demonstrated potential for improving patient outcomes and extending survival.
