Observation:

  • A patient presented with suspected brainstem stroke, undergoing DW-MRI and conventional MRI sequences.
  • MRI revealed extensive bilateral hyperintensities on T2-weighted images.
  • DW-MRI demonstrated diffuse hyperintensities in both hemispheres and a significant drop in apparent diffusion coefficient.

Findings:

  • Transtentorial herniation and brainstem compression were observed.
  • Absence of flow voids on T2-weighted images and intracranial vessels on MR angiography indicated cessation of blood flow.
