Background:

  • The cervicothoracic junction and upper thoracic spine (down to T4) are surgically accessible via an anterior approach using sternotomy.
  • The transsternal approach is optimal for resecting upper thoracic vertebral body lesions, facilitating interbody fusion and bone cement replacement.

Observation:

  • Modifications to the transsternal approach include manubriotomy with clavicle resection and partial lateral manubriotomy, offering limited lateral exposure but increased technical demand.
  • This study presents two cases of upper thoracic spine tumors operated on using a full median sternotomy.

Findings:

  • Full median sternotomy is technically simpler for thoracic surgeons and safer due to better mediastinal and great vessel exposure.
