Summary

Systemic capillary leak syndrome (Clarkson's syndrome) is a rare disorder causing fluid shifts. Early diagnosis and treatment with terbutaline and theophylline can be life-saving.

Background:

  • Systemic capillary leak syndrome (Clarkson's syndrome) is a rare idiopathic disorder.
  • Characterized by recurrent episodes of hypovolemic shock, hemoconcentration, and hypoalbuminemia.
  • Caused by a sudden shift of fluid and macromolecules from the intravascular to the interstitial space.

Observation:

  • A young man presented with recurrent hypotension and diffuse swelling.
  • Initial diagnosis was staphylococcal toxic shock syndrome.
  • Monoclonal gammopathy was an additional finding.

Findings:

  • The patient was diagnosed with systemic capillary leak syndrome.
  • Monoclonal gammopathy was associated with the syndrome in this case.

Implications:

  • Recognition of systemic capillary leak syndrome is crucial for prompt diagnosis.
  • Prophylactic treatment with terbutaline and theophylline may benefit patients.
  • Early intervention can mitigate this life-threatening condition.
