Summary

The St. Jude Medical mechanical heart valve with Silzone coating, which uses silver to prevent prosthetic valve endocarditis (PVE), showed no adverse effects in patients. Clinical data confirmed its safety and efficacy in preventing valve infections.

Background:

  • Prosthetic valve endocarditis (PVE) is a serious complication, often originating at the sewing cuff-annulus interface.
  • Elemental silver (Silzone coating) was applied to St. Jude Medical (SJM) mechanical heart valves to reduce PVE incidence.
  • In vitro studies demonstrated the antimicrobial properties of the Silzone coating.

Purpose of the Study:

  • To evaluate the safety and potential adverse effects of the Silzone silver coating in patients receiving SJM mechanical heart valves.
  • To assess serum silver levels and monitor for valve-related complications and endocarditis post-implantation.

Main Methods:

  • Serum silver levels were measured using graphite furnace atomic absorption spectrometry at multiple postoperative intervals (day 1, day 3, discharge, 1 month, 2 months).

  • 38 patients received SJM Masters Series valves with Silzone coating for aortic, mitral, or both valve positions between January and August 1997.
  • Follow-up data were collected to assess for hospital mortality, morbidity, and recurrent or new endocarditis.
  • Main Results:

    • No hospital mortality or valve-related morbidity was observed.
    • Serum silver levels peaked early post-surgery and subsequently decreased, remaining consistently below the normal threshold of 10 parts per billion (ppb).
    • Follow-up was 95% complete, with no instances of recurrent or new endocarditis reported.

    Conclusions:

    • The SJM Masters Series valve with Silzone coating demonstrated favorable clinical performance.
    • No adverse effects attributable to the silver coating were detected in the patient cohort.
    • The Silzone coating was associated with no valve-related complications and effectively prevented endocarditis.
