    Ocular abnormalities, or leukemic ophthalmopathy, affect 9% of children with acute leukemia. Central nervous system (CNS) prophylaxis is crucial to prevent leukemic invasion of the eye.

    Background:

    • Acute leukemia can manifest with ocular abnormalities, termed leukemic ophthalmopathy.
    • Ocular involvement in leukemia was observed in 9% of pediatric patients.
    • Previous treatment protocols did not include central nervous system (CNS) prophylaxis.

    Purpose of the Study:

    • To investigate the incidence and manifestations of leukemic ophthalmopathy in children with acute leukemia.
    • To evaluate the impact of CNS prophylaxis on the occurrence of leukemic ophthalmopathy.
    • To highlight the importance of ocular involvement in leukemia management.

    Main Methods:

    • Retrospective analysis of 657 children with acute leukemia treated with sequential single-agent chemotherapy.
    • Review of ophthalmologic findings, including retinal hemorrhage and leukemic infiltration.
    • Examination of cerebrospinal fluid for meningeal leukemia.

    Main Results:

    • Ocular abnormalities were detected in 52 children (9%).
    • Major manifestations included retinal hemorrhage (19 cases) and leukemic infiltration (29 cases).
    • Twenty-seven of 29 patients with available CSF data showed meningeal leukemia at the time of ocular symptoms; this was not observed in patients receiving CNS prophylaxis.

    Conclusions:

    • Leukemic ophthalmopathy is a significant complication in children with acute leukemia.
    • The absence of CNS prophylaxis is associated with a higher incidence of meningeal leukemia and ocular involvement.
    • Ocular surveillance and CNS prophylaxis, including the posterior eye, are essential for comprehensive leukemia treatment.
