Summary

Postpartum thyroiditis (PPT) affects 7.8% of women, often presenting with transient hormone changes. Early screening at six months postpartum is recommended, with ongoing monitoring for those with hypothyroidism to detect permanent thyroid dysfunction.

Area of Science:

  • Endocrinology
  • Obstetrics & Gynecology
  • Thyroidology

Background:

  • Postpartum thyroiditis (PPT) affects approximately 5% of women.
  • Understanding its incidence and clinical course in diverse populations is crucial.

Purpose of the Study:

  • To determine the incidence, clinical characteristics, and evolution of PPT in a Mediterranean population.
  • To identify optimal screening and follow-up strategies for PPT.

Main Methods:

  • A prospective study of 605 healthy pregnant women in the Mediterranean region.
  • Clinical and biological evaluations were conducted at multiple postpartum time points (1, 3, 6, 9, and 12 months).
  • PPT diagnosis was based on transient hyperthyroidism and/or hypothyroidism, with permanent hypothyroidism defined as overt and persistent after one year.

Related Experiment Videos

Main Results:

  • The incidence of PPT was 7.8% in the studied population.
  • Eighty-two percent of PPT patients exhibited hormone abnormalities by 6 months postpartum.
  • PPT manifested as hyperthyroidism and hypothyroidism (35.5%), hyperthyroidism alone (22.2%), or hypothyroidism alone (42.3%).
  • Permanent hypothyroidism developed in 0.82% of the total cohort (11.1% of PPT patients).

Conclusions:

  • Postpartum thyroiditis is more prevalent in the Mediterranean population than previously estimated.
  • Screening at 6 months postpartum can diagnose the majority of PPT cases.
  • Long-term follow-up is essential for hypothyroid PPT patients to identify permanent hypothyroidism.
