Summary

A woman with hypothyroidism experienced persistent low thyroid hormone levels due to levothyroxine (LT4) malabsorption. Parenteral LT4 therapy successfully normalized her thyroid hormone levels, indicating a viable treatment alternative for malabsorption.

Area of Science:

  • Endocrinology
  • Thyroidology
  • Pharmacology

Background:

  • Levothyroxine (LT4) is the standard treatment for hypothyroidism.
  • Subtotal thyroidectomy can lead to hypothyroidism requiring LT4 therapy.
  • Thyrotropin (TSH)-stimulating hormone suppressive therapy aims to maintain TSH levels below normal range.

Observation:

  • A 50-year-old woman presented with overt hypothyroidism despite LT4 treatment.
  • Laboratory results showed borderline low free thyroxine (FT4) and elevated TSH levels.
  • These levels remained unchanged with oral LT4 at 500 mcg daily.

Findings:

  • Investigations confirmed malabsorption of orally administered LT4.
  • Thyroid hormone levels normalized only after switching to parenteral LT4 therapy.

Related Experiment Videos

  • This highlights a significant challenge in managing LT4 absorption.
  • Implications:

    • Parenteral LT4 administration is a crucial alternative for patients with documented oral LT4 malabsorption.
    • This case underscores the importance of investigating absorption issues in non-responsive hypothyroidism.
    • Optimizing LT4 delivery is essential for achieving euthyroidism and preventing long-term complications.
