Summary

Cytologic features can distinguish proliferative breast disease with atypia (PBDA)/DCIS from nonproliferative breast disease (NPBD)/proliferative breast disease (PBD). However, differentiating NPBD from PBD using cytology alone is challenging.

Background:

  • Accurate classification of proliferative breast disease (PBD) is crucial for patient management.
  • Fine-needle aspiration (FNA) cytology can be limited by sampling errors when compared to histology.
  • Optimizing cytohistologic correlation is essential for reliable breast lesion assessment.

Purpose of the Study:

  • To evaluate specific cytologic features for differentiating various categories of proliferative breast disease.
  • To improve the accuracy of classifying breast lesions using cytopathology.

Main Methods:

  • Two smears (H&E and Diff-Quik) were prepared from 98 breast biopsies.
  • Smears were blindly reviewed for cellularity, background, cell groups, and nuclear features.

  • Cytologic classifications were compared with corresponding histologic sections.
  • Main Results:

    • Smears from atypia/DCIS showed higher cellularity, single cells, necrosis, nuclear overlap, and specific nuclear features (large nuclei, macronucleoli, pleomorphism, clumped chromatin, hyperchromasia).
    • Smears from atypia/DCIS were less likely to have complex cell groups, monolayers, and myoepithelial cells.
    • Smears from PBD showed larger and more complex cell groups compared to NPBD, but other features were similar.

    Conclusions:

    • Distinct cytologic features allow differentiation between nonproliferative/proliferative breast disease and proliferative breast disease with atypia/DCIS.
    • Few cytologic features effectively distinguish nonproliferative breast disease from proliferative breast disease without atypia.
