Summary

Understanding risk factors requires considering multiple pathways to health outcomes. Effect sizes for risk factors are reduced when multiple pathways exist, especially for odds ratios.

Area of Science:

  • Epidemiology
  • Biostatistics
  • Causal Inference

Background:

  • Risk factor association with causal intermediates versus endpoints.
  • Single pathway framework: identical relative risk estimates.
  • Multiple pathways: reduced relative risk estimates for endpoints.

Discussion:

  • Effect size reduction linked to proportion of endpoint cases via other pathways.
  • All effect measures are reduced in multiple pathways.
  • Reduction magnitude increases with proportion of alternative pathways.

Key Insights:

  • Single pathways: relative rate ratios and odds ratios are reduced.
  • Odds ratio reduction can be dramatic due to common causal intermediates.
  • Interpreting differences requires considering pathway multiplicity and intermediate prevalence.

Related Experiment Videos

Outlook:

  • Future research should account for pathway complexity in risk factor assessment.
  • Standardized methods for effect measure adjustment are needed.
  • Improved understanding will enhance causal inference in health research.
