Diffuse alveolar hemorrhage is a rare but severe complication of systemic lupus erythematosus (SLE). Prompt diagnosis and aggressive treatment, including managing infections, are crucial for patients with SLE-related lung bleeding.

Background:

  • Systemic lupus erythematosus (SLE) is a chronic autoimmune disease with potential multi-organ involvement.
  • Diffuse alveolar hemorrhage (DAH) is a rare but life-threatening pulmonary manifestation of SLE.

Observation:

  • Two cases of DAH in SLE patients are presented, both women aged 66 and 22.
  • Both patients had multi-organ SLE, including lupus nephritis, and experienced pneumonitis prior to DAH diagnosis.
  • DAH onset was characterized by respiratory failure, hemoptysis, acute anemia, and diffuse bilateral alveolar infiltrates on chest X-ray.

Findings:

  • Pulmonary hemorrhage initially responded to corticosteroid treatment but was ultimately fatal in both cases.
  • Autopsy revealed fibrosing alveolitis and alveolar bleeding in one patient and immune complex-induced alveolocapillary inflammation in the other.
