Summary

A rare tongue and neck tumor originated from Langerhans cells in a 37-year-old man. Electron microscopy revealed characteristic Langerhans cell granules, but S-100 protein was notably absent.

Background:

  • Langerhans cell tumors are rare, typically affecting the skin or lymph nodes.
  • Tumors originating from Langerhans cells in the oral and neck region are exceptionally uncommon.

Observation:

  • A case study of a 37-year-old male patient with a tumor in the tongue and neck area.
  • The tumor exhibited a polymorphocellular sarcoma morphology with distinctive bean-shaped, twisted nuclei.

Findings:

  • Electron microscopy confirmed the presence of Birbeck granules, indicative of Langerhans cell origin.
  • Ultrastructural analysis also identified tubulo-vesicular structures, ring-like plates, Golgi apparatus, lysosomes, and dendritic processes.
  • Tumor cells expressed total leukocytic antigen but lacked S-100 protein expression, an atypical finding for Langerhans cell neoplasms.

Implications:

  • This case expands the known anatomical sites for Langerhans cell-derived tumors.
  • The absence of S-100 protein challenges typical diagnostic markers and suggests potential heterogeneity in Langerhans cell neoplasms.
  • Further research is needed to understand the pathogenesis and diagnostic criteria for rare extranodal Langerhans cell tumors.
