Background:

  • Aneurysmal bone cysts (ABC) are benign, expansile bone neoplasms.
  • They commonly affect the spine and long bones, but are rare in the temporal bone.

Observation:

  • A pediatric patient presented with posterior auricular pain.
  • Imaging revealed an expansile lesion in the left petrous temporal bone causing cerebellar compression.

Findings:

  • Surgical excision via retrosigmoid-transmastoid craniectomy achieved total removal of the lesion.
  • Pathological examination confirmed the diagnosis of aneurysmal bone cyst.

Implications:

  • This case highlights the successful surgical and imaging management of a rare temporal bone ABC.
  • Aneurysmal bone cyst should be included in the differential diagnosis for bone neoplasms in the temporal region.
