Bladder-neck preservation during radical prostatectomy can reduce strictures and improve continence. This technique does not compromise cancer outcomes, even in high-risk patients.

Background:

Purpose of the Study:

  • To evaluate the impact of a bladder-neck preservation technique during total prostatectomy on oncologic outcomes and functional recovery.
  • To assess the risk of positive margins and anastomotic strictures with bladder-neck preservation.

Main Methods:

  • Analysis of 676 consecutive total prostatectomies.
  • Evaluation of tumor proximity to the bladder neck margin.
  • Correlation of bladder-neck margin status with preoperative prostate-specific antigen and Gleason score.

Main Results:

  • Bladder-neck preservation was associated with reduced risk of anastomotic stricture and faster return of urinary continence.
  • 4.3% of patients had tumor touching the bladder neck margin; only 1% had this as the sole positive margin.
  • Patients with positive bladder neck margins often had high preoperative prostate-specific antigen (>10) and Gleason score (≥7), indicating preservation did not compromise oncologic control.

Conclusions:

  • Bladder-neck preservation is a safe technique in radical prostatectomy, offering functional benefits without compromising cancer cure.
  • Extensive resection of the bladder neck is unlikely to improve curative rates and may increase morbidity.
