Area of Science:

  • Urology
  • Vascular Surgery
  • Andrology

Background:

  • Erectile dysfunction (ED) significantly impacts quality of life.
  • Penile prosthesis implantation is a common treatment for severe ED.
  • Alternative surgical options are sought for specific patient populations.

Purpose of the Study:

  • To evaluate the 4-year experience with deep dorsal vein arterialization.
  • To assess its efficacy as an alternative to penile prosthesis in young patients with pure cavernoocclusive dysfunction.
  • To report 3-year follow-up outcomes.

Main Methods:

  • A modified Furlow-Fisher operation was performed on 25 carefully selected young patients without risk factors for vascular disease.
  • Patients with arterial disease or abnormal penile electrodiagnostic findings were excluded.

Related Experiment Videos

  • Efficacy was assessed using subjective and objective evaluations, including the International Index of Erectile Function (IIEF) questionnaire.
  • Main Results:

    • Subjective and objective success rates were 70% and 60% at 3-year follow-up, respectively.
    • Significant improvements in IIEF scores for erection achievement and maintenance were observed (p<0.01).
    • Major complication (glans hypervascularization) occurred in 8% of patients; minor complications in 32%.

    Conclusions:

    • Deep dorsal vein arterialization is a preferable surgical option for highly selected young patients with pure cavernoocclusive dysfunction.
    • It provides a valuable alternative to penile prosthesis implantation in this specific group.
    • Careful patient selection is crucial for optimal outcomes.
