Background:

  • Transposition of the great arteries (TGA) with ventricular septal defect (VSD) is a complex congenital heart defect.
  • Arterial switch operation (ASO) is a common surgical repair for TGA.
  • Pulmonary artery banding (PAB) is sometimes used as a staged approach before ASO.

Observation:

  • A patient with TGA and VSD underwent PAB followed by ASO.
  • At 12 years of age, the patient developed severe neoaortic valve regurgitation.
  • Dilated aortic sinuses and poor leaflet coaptation were identified as the causes of regurgitation.

Findings:

  • Aortic valve repair was performed to address the neoaortic regurgitation.
  • Surgical techniques included subcommissural sutures, elliptical excision and reduction of aortic sinuses, and ascending aorta patch expansion.
