    Area of Science:

    • Ophthalmology
    • Surgical Innovation
    • Glaucoma Research

    Background:

    • Neovascular glaucoma presents significant challenges in intraocular pressure (IOP) management.
    • Current treatments often have limited long-term efficacy or significant side effects.

    Purpose of the Study:

    • To evaluate the safety and efficacy of a new surgical intervention for neovascular glaucoma.
    • To assess immediate and long-term outcomes of submuscular decompression multisclerectomy with 5-fluorouracil (5-FU) collagen implants.

    Main Methods:

    • A cohort of 29 patients with neovascular glaucoma underwent the novel surgical procedure.
    • Intraoperative complications and postoperative IOP, pain levels, and hypotensive effects were analyzed.
    • Patient outcomes were monitored for up to 2.5 years post-surgery.

    Related Experiment Videos

    Main Results:

    • The intervention achieved ophthalmic tone compensation in 96% of cases early postoperatively, decreasing to 63% in the remote period.
    • Painful syndrome was arrested in 96% of patients initially, with 65% experiencing sustained relief.
    • Intraoperative complications, including vitreous prolapse (13.8%) and hyphema (10.3%), occurred in 24.1% of cases.

    Conclusions:

    • Submuscular decompression multisclerectomy with 5-FU collagen implants demonstrates potential for IOP and pain management in neovascular glaucoma.
    • The procedure's hypotensive effect is attributed to increased ocular volume and enhanced uveoscleral outflow.
    • While initial results are encouraging, further studies are needed to confirm long-term effectiveness and refine surgical techniques.
