Summary

This study reports the first case of bilateral spontaneous cerebrospinal fluid (CSF) rhinorrhoea. Endoscopic repair of the CSF leak through bony defects was successful, highlighting a minimally invasive treatment option.

Annals of the Royal College of Surgeons of England·2018

Area of Science:

  • Neurosurgery
  • Otolaryngology

Background:

  • Cerebrospinal fluid (CSF) rhinorrhoea involves CSF leakage into the sinuses, potentially occurring spontaneously.
  • Spontaneous CSF leaks can arise from defects in the skull base, such as the cribriform plate.

Observation:

  • The case details bilateral spontaneous CSF rhinorrhoea, a rare presentation.
  • Bony defects on both sides of the cribriform plate were identified using endoscopic and radiological methods.

Findings:

  • Endoscopic techniques successfully identified and repaired bilateral bony defects causing CSF leakage.
  • The endoscopic closure of the CSF fistula was technically successful in this patient.

Implications:

  • Endoscopic repair offers a successful and minimally invasive treatment for spontaneous CSF rhinorrhoea.
