Area of Science:

  • Biochemistry
  • Neuroscience
  • Genetics

Background:

  • Creatine is essential for brain energy metabolism.
  • Disorders of creatine metabolism are rare but can cause severe neurological impairment.

Observation:

  • Two young sisters presented with mental retardation and severe language delay.
  • Standard metabolic and neuroimaging tests were unremarkable.

Findings:

  • In vivo proton magnetic resonance spectroscopy revealed generalized brain creatine depletion.
  • Diagnosis of a new creatine metabolism disorder was established.

Implications:

  • This highlights the utility of magnetic resonance spectroscopy in diagnosing rare neurometabolic disorders.
