Immunoadsorption (IA) therapy significantly increased soluble human leucocyte antigen-DR (sHLA-DR) levels in patients with autoimmune disease. This rise in sHLA-DR may contribute to the immunomodulatory effects of IA treatment.

Background:
- Immunoadsorption (IA) removes immunoglobulins (Igs) and immune complexes from blood.
- Igs are known to bind to human leucocyte antigen (HLA) molecules.
Purpose of the Study:
- To investigate the effect of Ig removal by IA on plasma concentrations of soluble HLA (sHLA).
Main Methods:
- Monitoring of sHLA class I (sHLA-I) and sHLA-DR plasma levels in nine patients with severe autoimmune disease undergoing IA.
- Plasma samples collected before, during, and after IA treatment (17 courses).
Main Results:
- A significant increase in sHLA-DR levels was observed after 2 liters of plasma adsorption (0.80 ± 0.10 µg/ml) compared to baseline (0.32 ± 0.05 µg/ml, p<0.001).
- sHLA-I levels showed only a minor increase (0.45 ± 0.06 µg/ml).
- sHLA levels returned to baseline within 24 hours post-IA.
Conclusions:
- The substantial rise in sHLA-DR during IA may play a role in the immunomodulatory effects of this therapy.
