Summary

D-dimer (DD) is often present in sarcoidosis patients and correlates with disease activity. Elevated DD indicates a higher likelihood of interstitial lung involvement in sarcoidosis.

Background:

  • Sarcoidosis is a multisystem inflammatory disease of unknown etiology.
  • D-dimer (DD) is a marker of coagulation activation, but its role in sarcoidosis is not well-defined.

Purpose of the Study:

  • To investigate the association between D-dimer (DD) status and indicators of disease activity in patients diagnosed with sarcoidosis.
  • To assess if DD levels correlate with radiographic findings, pulmonary function, and inflammatory markers in sarcoidosis.

Main Methods:

  • Prospective, observational study conducted in a tertiary care pulmonary clinic.
  • Included consecutive patients newly diagnosed with sarcoidosis.
  • Serum D-dimer (DD) levels were determined and correlated with clinical and laboratory parameters.

Main Results:

  • D-dimer (DD) was detected in 39.3% of the 28 study participants.
  • DD status showed significant correlations with radiographic disease stage (p=0.035), diffusing capacity of the lung for carbon monoxide (p=0.026), serum angiotensin-converting enzyme (ACE) levels (p=0.006), and dyspnea (p=0.034).
  • Patients with circulating DD were 3.8 times more likely to exhibit radiographic evidence of interstitial lung disease; however, DD was not associated with FVC or FEV1.

Conclusions:

  • D-dimer (DD) is frequently elevated in patients with sarcoidosis.
  • DD serves as a potential biomarker associated with sarcoidosis disease activity, reflecting radiographic findings, pulmonary function, and systemic inflammation.
