Summary

Piracetam did not significantly reduce death or disability in acute ischemic stroke patients. The drug showed no benefit for functional outcomes and may have a slight, non-significant increase in early mortality.

Area of Science:

  • Neurology
  • Clinical Trials
  • Pharmacology

Background:

  • Piracetam exhibits neuroprotective and antithrombotic properties.
  • These effects suggest potential benefits in reducing mortality and disability following acute stroke.

Purpose of the Study:

  • To evaluate the efficacy of piracetam in managing acute presumed ischemic stroke.
  • Assessing piracetam's impact on patient outcomes, including mortality and functional status.

Main Methods:

  • Systematic review of randomized controlled trials comparing piracetam with placebo.
  • Searched multiple databases (Medline, Embase) and contacted manufacturers for relevant studies.
  • Included trials with at least 48-hour entry criteria and reported mortality data.

Related Experiment Videos

Main Results:

  • Three trials with 1002 participants were analyzed; one trial dominated the data.
  • Piracetam showed a non-significant trend towards increased mortality (31%).
  • No significant differences were observed in functional outcomes, dependency, or mortality rates after adjusting for stroke severity.

Conclusions:

  • Evidence does not support piracetam's use in reducing dependency for stroke patients.
  • A potential unfavorable effect on early mortality was noted but may be due to baseline imbalances in stroke severity.
