Summary

Corticosteroids show no clear benefit or harm in acute traumatic brain injury (TBI). Further large trials are needed to determine if these widely available drugs offer any advantages for TBI patients.

  • Traumatic brain injury (TBI) is a significant cause of mortality and long-term disability.
  • Corticosteroids are frequently administered to patients with acute TBI.

Purpose of the Study:

  • To systematically evaluate the efficacy and safety of corticosteroid treatment in acute TBI.
  • To quantify the impact of corticosteroids on mortality, disability, and adverse events.

Main Methods:

  • A comprehensive literature search was conducted across major databases (MEDLINE, EMBASE, Cochrane Library) up to June 1999.
  • Included randomized controlled trials (RCTs) with adequate or unclear allocation concealment.
  • Data extraction focused on mortality, severe disability, infections, and gastrointestinal bleeds.

Main Results:

  • Nineteen trials involving 2295 participants were analyzed.
  • Corticosteroids did not significantly alter the risk of death (pooled RR 0.96; 95% CI 0.85-1.08).
  • No significant effects were observed for death or severe disability (RR 1.01), infections (RR 0.94), or gastrointestinal bleeding (RR 1.11).

Conclusions:

  • The current evidence does not exclude moderate benefits or harms from corticosteroid use in acute TBI.
  • Given the prevalence of TBI and the accessibility of corticosteroids, large-scale, simple trials are warranted to definitively assess their therapeutic value.
