Summary

Hyperbaric oxygen (HBO) does not appear to reduce neurological symptoms after carbon monoxide (CO) poisoning. More research is needed to determine if HBO has a role in treating CO poisoning.

Background:

  • Carbon monoxide (CO) poisoning is a significant global health concern.
  • Previous studies suggested hyperbaric oxygen (HBO) may prevent neurological issues.
  • HBO is widely used for CO poisoning despite limited evidence.

Purpose of the Study:

  • To evaluate the effectiveness of HBO versus normobaric oxygen (NBO) in preventing neurological symptoms after acute CO poisoning.

Main Methods:

  • Systematic review of randomized controlled trials (RCTs) from major databases.
  • Included non-pregnant adults with acute CO poisoning.
  • Pooled odds ratio calculated for neurological symptoms at one-month follow-up.

  • Six RCTs were analyzed, varying in quality and CO poisoning severity.
  • Neurological symptoms occurred in 34.2% of patients treated with HBO versus 37.2% with NBO.
  • No statistically significant benefit of HBO over NBO was found (OR 0.82; 95% CI 0.41-1.66).

Conclusions:

  • Current evidence does not support the routine use of HBO for preventing neurological symptoms in unselected CO poisoning patients.
  • Existing RCT evidence is insufficient to guide clinical practice.
  • Further high-quality, multicenter, randomized trials are necessary to clarify HBO's role in CO poisoning management.
