Background:

  • Left ventricular assist system (LVAS) placement is associated with frequent thromboembolic and hemorrhagic complications.
  • Platelet activation and function are implicated in these adverse events following LVAS implantation.

Observation:

  • Flow cytometry analysis revealed increased expression of CD62P and CD63 on platelets post-LVAS placement.
  • Annexin V binding, indicating membrane asymmetry, was not elevated, suggesting specific activation pathways.

Findings:

  • LVAS implantation led to suppressed thrombin activation, which normalized after heart transplantation.
  • Heparin significantly suppressed thrombin activation, while aspirin and dipyridamole had no effect.
  • Protamine sulfate and heparinase attenuated the observed thrombin suppression.
