Summary

Trigeminal neuralgia can be caused by sinusitis, not just vascular compression. Treating sinusitis with antibiotics resolved severe neuralgic pain in one patient, avoiding surgery.

Background:

  • An elderly woman presented with severe left-sided trigeminal neuralgia following an upper respiratory tract infection.
  • Magnetic Resonance Imaging (MRI) revealed severe sinusitis but no intracranial pathology.

Findings:

  • Antibiotic treatment for sinusitis led to complete resolution of the patient's trigeminal neuralgia symptoms.
  • This case suggests sinusitis as a potential, treatable cause of trigeminal neuralgia.

Implications:

  • Considers sinusitis as a differential diagnosis for trigeminal neuralgia.
  • Highlights the importance of medical management for certain TN cases before surgical consideration.
  • Suggests a non-compressive etiology for trigeminal neuralgia that responds to antibiotics.
