Summary

Natural coral bone grafts show slow, centripetal resorption in humans, with limited bone replacement over two years. Complete resorption and defect restoration were not observed in this study.

  • Natural coral (NC) is explored as a bone substitute.

Purpose of the Study:

  • To evaluate the resorption rate and pattern of Porites natural coral implants in human iliac crest defects.
  • To assess the extent of new bone formation and replacement of NC implants.

Main Methods:

  • Bicortical bone grafts from 10 patients were used to fill defects, implanted with NC blocks.
  • Radiographic (X-ray) and quantitative CT scans monitored changes over a mean of 2.1 years.
  • Biopsies were obtained at 1 year for histological analysis.

  • NC blocks exhibited centripetal resorption but remained detectable by imaging throughout the study.
  • Implant density did not change; 70% of implants were <50% of original size by study end.
  • Bone ingrowth was minimal, observed in only 2 of 7 biopsies; one implant was removed due to infection.

Conclusions:

  • Natural coral resorbs centripetally, with faster resorption potentially linked to tissue ingrowth.
  • NC implants did not fully resorb, nor did they restore the iliac crest defect within the study period.
